As per Maldivian law a diving insurance is mandatory. If you do not hold a dive insurance, please select one from the below DAN Sports Diver Short Insurance plan durations and accept the listed premium fees:

This is a statement in which you are informed of established conditions of participation in our activities. Your signature on this statement is required. Read and discuss the statement prior to signing. If you are a minor (below 18), this form also must be signed by a parent or a guardian.


CONDITIONS OF PARTICIPATION

  1. Sub Oceanic, its employees and Le Meridien Maldives herewith decline all liability for personal injury, property damage and wrongful death arising from the participation in any snorkelling and scuba diving activities.

  2. Every participant must strictly follow the directions of the Guide / Instructor / Divemaster. Otherwise he / she may be debarred from any activity without being entitled to a refund.

  3. The participant is liable for damage to, or loss of, hired materials, whereby Sub Oceanic is entitled to charge for the loss of earning in condition to repair or renewal costs.

  4. The participation in all diving and snorkelling activities organized by Sub Oceanic is at the participants own risk. The participant expressly confirms that he / she is physically fit and has no knowledge of any medical reason which make him / her unsuited for diving or snorkelling. Sub Oceanic is entitled, but not obliged, to require a medical certificate.

  5. The purpose of the medical conditions listed below is to find out if you should be examined by a physician before participating in diving activities. A positive response to a question does not necessarily disqualify you from diving. A positive response means that there is a pre-existing condition that may affect your safety while diving and you must seek the advice of a diving physician prior to engaging in any dive activities. A written medical statement is required if any of these items apply to you:
    Pregnancy, Recurrent back problems, Frequent colds, sinusitis or bronchitis, Back or Spinal injury, Any form of lung disease, Diabetes, Pneumothorax (collapsed lung), Injuries or fractures, Epilepsy, seizures or convulsions, High Blood pressure, Chest disease or surgery, Heart Disease or Heart attack, Angina, heart surgery or blood vessel surgery, Ear or Sinus Disease/surgery, Recurring complicated migraine headaches, Hearing loss or problem with balance, Blackouts or fainting, Recurrent Ear problems, Any dive accidents or Decompression illness, Hernia, Head injury with loss of consciousness (past 5 yrs), Colostomy or ileostomy, Bleeding or other blood disorders, Dysentery or Dehydration requiring medical, Ulcers or ulcer surgery intervention, Asthma, Behavioral health, mental or psychological problems, Recreational drug use or treatment for (panic attack, fear of closed or open spaces) or alcoholism in the past 5 years

  6. The participant declares that he / she do not suffer from any above, and that no medical reasons exist against scuba diving and snorkelling. Furthermore, there are, to his / her knowledge, no other reasons on which the practice of scuba diving/snorkelling could have a detrimental affect on his / her health.

    If the participant is presently taking any prescription medication or if he/she is above the age of 70 years a written medical statement is mandatory. It is within the participant responsibility to supply such a document.

  7. The participant is aware that diving sport safety requires the diving equipment to be in perfect conditions and working order, Sub Oceanic regularly controls the conditions of the Dive Centre’s diving material. Nevertheless, the participant should check that all the equipment is in good working order, and report eventual defects. The participant bears complete responsibility for private gear.

  8. The participant agrees to follow the Diving and Snorkelling Regulations and General Safety Rules displayed in the Dive Centre and detailed by PADI. & SSI. Violations may lead to a ban from further activities, without being entitled to a refund.

  9. A surface interval of 18 - 24 hours before departure by plane is mandatory.

  10. Equipment rental must be returned to the Sub Oceanic staff during opening hours 09:00-18:00, within rental frame and not later than 2 hours prior to departure from the island. Equipment not returned will be considered as lost and will be charged accordingly.

  11. Please note that we cannot be held responsible for any damage or loss of our guest’s own equipment whilst it is in our care.

  12. For Nitrox users, personally analyse and determine the O2 partial pressure and the maximum operating depth.

  13. Kindly note our Cancellation policy: all activities cancelled after opening hours on the day before will be charged 50% of the price, cancellations on the same day will be charged 100%

I have read the above statement and I have any questions answered to my satisfaction. I understand that the participation in all diving and snorkelling activities takes place at my own risk and that Sub Oceanic Ventures Pvt Ltd. and its staff cannot be held responsible for personal injury, property damage (personal and rented), and / or wrongful death arising from use of rental equipment. This also applies when I travel in the diving or snorkelling boat, dive/snorkel together with the instructor or my dive snorkelling buddy.I accept the Conditions of Participation and have fully informed myself of the contents of this Liability Release and Assumption of Risk Agreement by reading it before I signed it on behalf of myself and my heirs.


PHOTOGRAPHY/VIEDEO RELEASE Participant hereby grants to Sub Oceanic, its representatives, and employees the right to take photographs/videos of the participant in connection with participant's participation in the activities. Participant hereby authorizes the Sub Oceanic to copyright, use and publish the same in print and/or electronically. Participant hereby agrees that the Host may use such photographs of participant for any lawful purpose, including but not limited to publicity, illustration, advertising, and Web content. By providing my contact details, I agree to receive messages from Sub Oceanic.
